REQUIRED DOCUMENTS FOR LOCAL STUDENT APPLICANTS FOR COLLEGE PROGRAM

FRESHMEN (NEW ENROLLEES)

  1. Accomplished Application Form
  2. High School Report Card (Form 138)
  3. High School Transcript (Form 137a)
  4. Certificate of Good Moral Character
  5. Original NSO birth Certificate
  6. Marriage Contract, if married ( duly authenticated by NSO)
  7. 5 COPIES 2×2 picture
  8. 2 pcs. long white Folder
  9. 2 pcs. long plastic Brown envelop

TRANSFEREES

  1. Accomplished Application Form
  2. Honorable Dismissal/ Transfer Credentials
  3. Temporary Transcript/ Certificate of Grades for evaluation purposes
  4. Official Transcript of Records to be sent by the previous school with a notation:

COPY FOR: AGO MEDICAL AND EDUCATIONAL CENTER, Rizal St., Legazpi City

  1. Original NSO Birth Certificate
  2. Marriage Contract, if married ( duly authenticated by NSO)
  3. Subject description taken from the previous school
  4. 5 COPIES 2×2 picture
  5. 2 pcs long white folder
  6. 2 pcs. long plastic brown envelop

Admission Procedure

STEP 1
Proceed to the Registrar’s office for Application Form for Admission and Preliminary Form

  • Fill up Application Form for admission and basic data on the Preliminary Form
  • Evaluation of Required Credentials
  • Proceed to Cashier and Pay application Fee (Application fee is nonrefundable)

 

STEP 2
Proceed to the College Dean’s Office for interview and Evaluation of Academic requirements for enlistment of subjects to be taken during the semester.

 

STEP 3

Proceed to Registrar’s office for confirmation of approved listed subjects to be taken during the semester

 

STEP 4

Submit the duly approved Preliminary form by the Dean and Registrar to the Assessment Section

 

STEP 5

Proceed to Accounting office for Assessment approval
STEP 6

Proceed to Cashier, pay the necessary fees.

 

STEP 7

After paying the fees and enlistment of subjects, you will be officially enrolled

Get a copy of your registration form which shows all the subjects
that you will need to take during the semester.

  1. Accomplished Application Form
  2. Original Copy of NMAT
  3. Photo copy of Diploma
  4. Official Transcript of records with notation:

Copy for: BICOL CHRISTIAN COLLEGE OF MEDICINE, Rizal St., Legazpi City

  1. Honorable Dismissal
  2. Special Order
  3. Subject description taken from the previous school (if: transfer student)
  4. Certificate of Academic Standing with General Weighted Average (GWA)
  5. Certificate of Good Moral Character from Dean of Student Affairs or from two (2) professors from college.
  6. 6 pcs. 2×2 picture with white background
  7. 2 pcs Long brown envelop
  8. 2 pcs. Long white folder
  9. 2 pcs documentary stamp
  10. Original NSO Birth Certificate
  11. Marriage Contract, if married ( duly authenticated by NSO)
  12. Letter of intent – if Scholarship applicant
  13. Latest Income Tax Return (ITR) of parents – if Scholarship applicant
